Only a valid holder of a C-15 Certificate of Fitness certificate is able to distribute the Affidavit for Flame Retardant Treatment; the FDNY recognizes no other. Once a business owner receives this affidavit, he or she needs to file it with the Bureau of Fire Prevention within 10 days. A paper copy of this affidavit needs to remain on site as well. And to add to your paper file, every time the treated items are washed or dry-cleaned, a record of that needs to be kept as well.
A valid Affidavit for Flame Retardant Treatment should have a few particular things on it in order for it to be acceptable to the FDNY. On this document, you should see the following: the date of the flameproofing treatments, the name of the person/company performing the treatments, their C-15 Certificate of Fitness holder number, the expiration date of their certificate, details of the items and their treatment, the names of chemicals and the manufacturers as well as the Certificate of Approval numbers, and finally a statement indicating that FDNY requirements were followed and the treated materials passed the field flame test.
